Job Title: Complex Care Assessor
Location: Home based with national travel across England
Working pattern: 37.5 hours per week
Salary: £45,000 Per Annum

The Role
As a Complex Care Assessor, you will play a critical part in the referral and transition journey:
* Carry out high-quality assessments (face to face and virtual) for adults with learning disabilities, autism, mental health conditions, and associated complex needs
* Write clear, person-centred reports that highlight needs, risks, and positive outcomes
* Recommend practical, tailored solutions – from risk management to staffing and training
* Work closely with operations teams, home managers, Positive Behaviour Support and Learning and Development to support decision making and plan safe and successful move-ins
* Supporting smooth, well-planned transitions for the people we support
* Acting as a trusted voice for colleagues and external partners
* Helping us continually improve how we assess, learn, and grow
About You
* Experience in assessing individuals with very complex presentations who require support to live a meaningful life in their community
* Confident in completing high- quality assessments and writing detailed reports
* Knowledge of the Mental Health Act 2025 and current best practice
* A collaborative approach and strong communication skills
* Organised, able to meet deadlines, and comfortable working across multiple teams
* A proactive problem-solver with an “art of the possible” mindset
* Registered RMN or RNLD with a valid NMC PIN is desirable
* Full UK driving licence and willingness to travel
