Location: Chesterfield, S41 7SL (hybrid working)
Shifts: Full-time, 37.5 hours per week
Salary: Competitive, dependent on experience
About the Role
We have a fantastic opportunity here at Ivolve Care & Support for a Referral Support Officer to join our growing team. The Referral Support Officer will work closely with a team of Partnership Development Managers, our services and a wide range of stakeholders to support the referral process, ensuring individuals experience a smooth, person-centred move into our care.
Key Responsibilities
- Monitor and manage the referrals inbox, ensuring all enquiries are logged on HubSpot, actioned in a timely manner and shared with the appropriate Partnership Development Manager
- Work closely with Partnership Development Managers to obtain all relevant referral information from referrers, following up requests where there are delays
- Coordinate and schedule assessments for each person we support
- Maintain accurate, up-to-date records across all systems and databases.
- Complete tenancy sign-ups for Supported Living services, liaising with housing providers, social workers, the people we support and their families, obtaining all key information required.
- Coordinate the ‘move in checklist’ to ensure all actions are completed prior to a person we support moving in
- Note taking at key meetings throughout the referral process
- Support obtaining positive news stories from our services
- Monitor any vacant bedrooms, highlighting those that require ‘dressing’ so they are ready to showcase
- Assist in the preparation of reports and presentations for senior leadership as required
Skills and Experience
- Strong administrative skills with Proficiency in Microsoft Office (Word, Excel, PowerPoint, Outlook)
- Excellent interpersonal skills, with the ability to build strong relationships with different functions across the organisation and external stakeholders.
- Strong organisational skills and the ability to manage multiple priorities and deadlines effectively.
- An understanding of health and/or social care and experience in a similar role within health and/or social care would be advantageous
- Solutions focussed and ability to problem solve
- A collaborative team player who is also comfortable working independently.
- Access to a vehicle for occasional travel
