Role Overview
The Nurse Assessor is responsible for completing high-quality pre-move in assessments for individuals referred to ivolve’s supported living and residential services. The role ensures that risks are clearly understood and mitigated and that services are able to safely and successfully meet needs from day one. The postholder plays a critical role in supporting move ins, reducing placement breakdowns and enabling ivolve to fulfil it’s aim to benefit more lives.
Responsibilities include:
- Completing comprehensive face to face and virtual pre-move in clinical assessments for adults with learning disabilities, autism, mental health needs, physical disabilities and/or complex health needs, within agreed timeframes.
- Working collaboratively with operational teams and home managers to support decision making and facilitate safe move ins
- Write high-quality assessment reports with clinical clarity, clearly setting out needs, risks and outcomes while highlighting the benefits of choosing ivolve in line with organisational standards.
- Recommending reasonable adjustments including risk mitigation strategies, staffing model changes, training requirements and additional specialist input where required
- Working alongside the PBS and L&D teams to support training and offer advice to operational teams on clinical risk and complex needs
- Acting as a credible clinical point of contact for external stakeholders
- Providing clinical input into transition planning
- Contribute to the development of assessment tools, care pathways and best practice
- Use data and learning from assessments to improve referral conversion and placement sustainability

Essential skills
- An experienced RMN or RNLD with a valid NMC Pin Number, you’ll have;
- A strong track record within learning disability and/or mental health services
- Experience in producing accurate assessments and high-quality reports with good attention to detail
- Excellent communication skills
- Ability to work to deadlines
- Strong problem-solving ability with an ‘art of the possible’ mindset
- The ability to work as part of multiple, cross functional teams.
- Knowledge of the Mental Health Act 2025 and the latest nursing practices
- Kindness, passion and resilience with a desire to support ivolve to benefit more lives
- A valid driver’s licence and use of a car with the ability to travel across England
