About the Role
We have a fantastic opportunity for a Referral Support Officer to join our growing team at ivolve. You’ll work closely with Partnership Development Managers, our services and a wide range of stakeholders to support the referral process, ensuring every individual experiences a smooth, person‑centred transition into our care.
This role is key to ensuring accurate information flow, timely coordination and a high‑quality experience for the people we support and their families.
Key Responsibilities
- Monitor and manage the referrals inbox, ensuring all enquiries are logged and actioned promptly
- Work closely with Partnership Development Managers to gather all relevant referral information
- Coordinate and schedule assessments for prospective individuals
- Maintain accurate and up‑to‑date records across systems and databases
- Complete tenancy sign‑ups for Supported Living services, liaising with housing providers, social workers, families and the people we support
- Coordinate the move‑in checklist to ensure all actions are completed before someone joins our service
- Take notes at key meetings throughout the referral process
- Support the collection of positive news stories from services
- Monitor vacant bedrooms and highlight those requiring preparation for viewing
- Assist with preparing reports and presentations for senior leadership
Skills & Experience
- Strong administrative skills with proficiency in Microsoft Office (Word, Excel, PowerPoint, Outlook)
- Excellent interpersonal skills with the ability to build strong relationships internally and externally
- Highly organised with the ability to manage multiple priorities and deadlines
- Understanding of health and/or social care is advantageous
- Solutions‑focused with strong problem‑solving skills
- A collaborative team player who can also
- work independently
- Access to a vehicle for occasional travel
